Such activities shall be performed only by experienced and trained employees selected for such work by the competent person.     (a)    Location: 209 Collins Street, Newton, MA   On or about 10/23/13, Scaffolds were not erected under the supervision and direction of a competent person.

(a)    Location: 209 Collins Street, Newton, MA  On or about 10/23/13,  each employee working on a pump jack scaffold was not provided training to recognize the hazard associated with that scaffold.
