29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     On 10/23/13, employees were accessing a ladder jack scaffold and a section of the roof with step ladders that did not extend at least 3' above the landing surface.

29 CFR 1926.1053(b)(3): Ladders were loaded beyond the maximum intended load for which they are built:   On 10/23/13, employees were working from a ladder jack scaffold that was supported by a Class II aluminum ladder that had been separated into two pieces. The load of two workers and the plank are approximately 450 lbs. while the ladder is rated at 225 lbs. when used as intended.
