29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about October 24, 2013, seven employees were installing corrugated metal roofing panels on the roof of the building under construction without utilizing fall protection, exposing the employees to a fall hazard of approximately 20 feet in height to the concrete foundation below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about 24 October, 2013, seven employees were installing corrugated metal roofing panels on the roof of the building under construction without the employer providing fall protection training, exposing the employees to a fall hazard of 20 feet onto the concrete floor below.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step.  On or about 24 October, 2013, seven employees were performing framing activities on the roof of a building under construction. Two of the employees used the top step of a Werner, 9-foot, portable, free-standing ladder to access the second level, exposing the employees to a fall hazard of approximately 9 feet and 4 inches unto the concrete foundation below.

On or about 24 October 2013, two employees were performing framing activities on a building under construction while carrying and cutting corrugated metal roofing panels, without using gloves, exposing the employees to the hazard of being cut by the thin sheets of metal.
