29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): a) At the site, on or about 10/28/2013, the working level of tubular welded frame scaffolding, approximately 10 feet above the ground, was not fully planked. A gap of approximately 20 inches existed between the platform and the uprights, exposing employees to fall hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access: a) At the site, onor about 10/28/2013, a ladder was not provided to allow employees access to the working levels of the tubular welded frame scaffolding, approximately 21 feet above the ground, causing employees to use the cross-bracing and end frames as a means of access, exposing employees to fall hazards.
