29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Dillon Companies dba Baker's Supermarkets: The employer failed to develop an adequately complete energy control program as required by this standard. In that the employer has not developed equipment specific procedures for de-energizing as well as the employer has not provided training to its employees which are Authorized, Affected and Other.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Meat Department of Twin Creek ; Baker's Supermarket 3614 Twin Creek Dr., Bellevue, NE 68123: The employer failed to ensure that employees were not exposed to the rotating parts and cutting blade of a Hobart Meat Saw ( Serial# 27-07-12 / Model# 6614) during the cleaning and/or sanitizing operation. The guards on the saw had been removed and/or opened to allow the employee to thoroughly clean / sanitize the saw ; the employer did not ensure that employee disconnected the electricity from the saw prior to commencing.
