29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a)  At the jobsite, on or about 10/23/13:  An employee was using a Dewalt circular saw, with a three pronged connection to the power source, where that cord was pulled free from the plug.
