29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:    a)  Intoxx Fitness, 123 Leverington Avenue, Philadelphia, PA, Cardio Loft- Two energized electrical outlet boxes used to power televisions and exercise equipment did not have faceplates exposing employees to electrical shock hazards while connecting equipment and working in the area. Observed on or about October 29, 2013.    Note:  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.305(c)(4): Faceplates for flush-mounted snap switches. Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface:   a)  Intoxx Fitness, 123 Leverington Avenue, Philadelphia, PA, Top of entry stairs- The box containing three switches controlling general lighting in the gym was not provided a faceplate exposing employees to energized conductors when turning on and off the lights and using the hallway/entryway. Observed on or about October 29, 2013.     Note:  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a)  Intoxx Fitness, 123 Leverington Avenue, Philadelphia, PA, Front Desk Area- An electrical box with knock-outs for accepting fittings was laying on the floor below the stainless steel sink. The box was energized and in use as an outlet box to power equipment behind the Front Desk.  In accordance with listing and labeling instructions this type of electrical box is to be rigidly mounted in or on a wall. Observed on or about October 29, 2013.  Note:  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
