29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Jobsite: On or about 10/29/13, employees were exposed to fall hazards where the ladder did not extend 3' above the point of egress to the roof.

29 CFR 1926.1060(a): The employer shall provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.  Jobsite: On or about 10/30/2013, the employer failed to provide adequate training to employees using the ladders.
