29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:     On or about October 29, 2013, in front and to the rear of the house, the top-plate and outer edges of the trusses; three employees were observed working on 5:12 pitched roof trusses without utilizing any fall protection system. The employees were exposed to a fall hazard of greater than six feet.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:     On or about October 29, 2013, inside the house under construction, electrical extension cords and an Electric Air Compressor had broken strain relief in the following instances:      a)    Rolair Systems, Wheeled Electric Air Compressor, electrical cable (male end).    b)    A 100-foot yellow extension cord (male end) used to power the DeWalt 7 ¼-inches Circular Saw.     c)     A 100-foot red extension cord (male end) used to power the Hitachi 7 ¼-inches Circular Saw.
