29 CFR 1926.651(e): Employee was not prohibited to be underneath loads handled by lifting or digging equipment: On or about October 29, 2013, at the Ventanas Unit 6 construction project, two employees were observed underneath a suspended precast concrete drainpipe weighing approximately 4520 pounds being lowered into the trench by a CAT 320DL hydraulic excavator. This condition exposed the employee to crush-by and struck-by hazards.

This condition exposed the employees to entrapment and/or cave-in hazards

Employees in the trench were not protected from cave-ins by an adequate protective system such as trench box. This condition exposed the employees to entrapment and/or cave-in hazards.
