29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose:     On or about October 29, 2013, inside of the apartment complex under construction employees were using a hand-made electrical handy-box with a single duplex receptacle (15A, 125VAC, 2-pole, 3-wire) to operate their power tools. The handy box with no cover exposed the electrical bare conductors. Employees were exposed to electrical shock hazards.

29 CFR 1926.405(g)(1)(i): Flexible cords and cables were not suitable for conditions of use and location:    On or about October 29, 2013, inside the apartment complex under construction, the employer did not ensure that employee(s) followed manufacturer's instructions. Electrical equipment was not protected from possible damage; employees were exposed to electrical shock hazards in the following instances:      a)    Employees had three electrical extension cords plugged together.     b)    A black extension cord spliced into an orange extension had outer insulation damage.     c)    A 100-foot red extension cord was run through a window without providing protection.
