29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:       (a) On, and at certain times before, October 2, 2013, employees were exposed to fire and smoke hazards in that an excessive level of volatile fibrous materials had been allowed to accumulate inside and around the ventilation duct work for calendar #5 in the production area of the plant.  On October 2, 2013, a fire was started inside the duct work serving calender #5 when a worker cut into the duct work to create an access door. 29 CFR 1910.38(e): 29 CFR 1910.38(e): The employer did not train employees to assist in a safe and orderly evacuation of other employees.       (a) On or about October 2, 2013, employees were unnecessarily exposed to fire and smoke inhalation hazards due to their failure to execute a timely and complete evacuation during a fire event.  Numerous employees were inside the plant and on the roof of the plant long after the fire was past its incipient stage.  The employees, who were not in any type of protective gear, had to be ordered out of the building and off of the building's roof by the responding fire authorities.      Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, or flying chips and sparks:       (a) On or about November 5, 2013, employees were exposed to the following machine hazards:     (1) The lower pulley on the Vertical Sag ("Big Baboon") machine was unguarded exposing the operator to possible finger or hand injuries (including amputation) in the event the operator's fingers or hand got caught between the lower pulley and the in-running belt.     (2) The circular blades ("knives") on the back of calender #5 were unguarded exposing the operator to possible finger amputation.     (3) The large, powered industrial sliding door to the Tech Center was discovered to be in its "Auto" mode which resulted in its closing automatically after it had been open a brief period of time.  The door lacked any type of sensing device or safety feature to prevent an employee from being caught between the door and its frame as it automatically returned to its closed position with considerable force.     (4) The rotating axle hubs near the floor on the Horizontal Bias #2 machine exposed employees to entanglement hazards.       Abatement certification is required for this Item.
