29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:    a) PR-Weld & Manufacturing, Ltd. - West Unity, Ohio: On or about October 31, 2013, the employer did not assure that the Hyster fork truck (model E60XM2-33; s/n F108V29634A) was marked to include the boom front-end attachment to show the approximate weight of the fork truck and attachment combination at maximum elevation with the load laterally centered.  The lifting capacity of the fork truck using the boom was not known, leaving potential for tip-over hazards.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a) PR-Weld & Manufacturing, Ltd. - West Unity, Ohio: On or about October 31, 2013, employees operated powered industrial vehicles to transport steel products in the storage and saw areas.  The employer did not assure that employees successfully completed a forklift operator training program containing elements in 29 CFR 1910.178(l) before employees were permitted to operate forklifts.

PR-Weld & Manufacturing, Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(3)(ii), which was contained in OSHA Inspection Number 311608780, citation number 1,  item number 2, and was affirmed as a final order on July 9, 2009, with respect to a workplace located at 1910 Industrial Dr., Bryan, Ohio 43506.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  a) PR-Weld & Manufacturing, Ltd. - West Unity, Ohio: On or about October 31, 2013, the employer failed to assure that approval was obtained by the manufacturer before adding a working platform to the Hyster fork truck (model E60XM2-33; s/n F108V29634A).  The addition was used to lift workers who conducted painting operations.
