29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:     (a) Printing Press Area, West Mezzanine; A ladderway floor opening on the second level mezzanine was not covered or guarded. The floor opening was 15-3/4 inches wide by 16 inches wide and had a fall distance of 8 feet to the lower level.

Duct tape was placed over the cracked plug end.
