29 CFR 1904.29(b)(1):  A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation:  (a) Buy-Rite Liquidators, Inc., 1076 Park Road, Blandon, Pennsylvania - The employer did not maintain an OSHA 300 log for calendar year 2013.  An injury that meets the requirements for recordability has occurred.  On or about October 8, 2013, an employee sustained a fractured left foot and leg after getting struck by a forklift in the Sorting area, which resulted in days away from work.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):  (a) Buy-Rite Liquidators, Inc., 1076 Park Road, Blandon, Pennsylvania - The employer provided refresher training and evaluation of a forklift operator involved in an incident on October 8, 2013 that resulted in personal injury to another employee, and the employer did not certify this refresher training and evaluation.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
