OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:         On or about November 1, 2013, employees were exposed to fall hazards while working in an elevated work platform, up to 14 feet high, attached to the forks of a CAT TH360B Telehandler.  The platform was 12 feet wide.  The width of the platform extended greater than ANSI standard maximum of 10 inches past the load bearing tires on each side.     Among other means, feasible and acceptable methods to correct the hazard would be to follow ANSI/ITSDF B56.6 - 2011 Safety Standard for Rough Terrain Forklift Trucks including but not limited to:  1.    Use a work platform that does not extend more than 10 inches on each side past the load bearing tire.   2.    Use an alternative means to elevate employees to working levels.

29 CFR 1926.1053(b)(1):      Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:           Employees were exposed to fall hazards of approximately 14 foot high to a lower level while using a Werner fiberglass ladder, which did not extend 3 feet above the upper landing surface being accessed, or the roof surface did not have a grasping device to provide assistance to the employee in mounting and dismounting the ladder.
