29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a)  At 1710 Crawford Rd Phenix City, Alabama:  On or about October 30, 2013 and at times prior;  the employer failed to ensure employees working from an elevated bucket-type aerial lift were wearing a harness and attached to the bucket exposing them to fall hazards of 30 feet to the ground below.
