29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Jobsite: On or about November 1, 2013, Three employees were not properly tied off while working on a roof. Two employees were tied off at the hip and a third was tied off in front. In addition the employee that was tied off in the front was not wearing a full body harness but rather a seat type harness.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill: (a) Jobsite: On or about November 1, 2013, the employer needs to retrain his employees so that they can recognize and minimize fall hazards and in how to properly wear personal fall protection equipment when working from a roof.

29 CFR 1926.1053(b)(11): Ladders were moved, shifted, or extended while occupied: (a) Jobsite: On or about 11/1/2013 an employee climbed the 32 foot aluminum extension ladder and pushed the top 1/2 of the ladder up to lower it. He continued to do this until the top part of the ladder was near the ground. There was no rope on the extension ladder to use to raise the top part of the extension ladder so that it could be lowered. The fall hazard to the employee was about 8 feet to the ground.
