29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasion, and openings through which conductors enter were not effectively closed. On or about November 1, 2013 the employer did not ensure that conductors entering an electrical starter box powering an automotive lift were protected from abrasion and/or that the conductors were properly secured to the connection point. This condition exposed employees to hazards associated with electrical shock.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): On or about November 1, 2013 the employer did not ensure that flexible cord(s) were inspected before use on any shift and removed if there is a defect or evidence of damage that might expose an employee to injury. A flexible cord was found to be defective; the cord's ground prong was missing. This condition exposed employees to hazards associated with electrical shock.
