29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:    South-east pressroom, mechanical power presses: on or about November 5, 2013, employees locking out production machinery for servicing, preventative maintenance, or other setup tasks were not inspected periodically (at least once per year) to ensure their level of proficiency in the hazardous energy control procedures implemented by the employer to ensure sufficient level of protection and compliance.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) Toolroom, Roll-In S-2 bandsaw: on or about November 5, 2013, employees cutting variety of metal stock were exposed to amputation hazards where the unused portion of the running blade was not guarded.    There is no abatement certification or documentation required for this item (a). Abatement documentation is required for this item (b) in accordance with 29 CFR 1903.19(d).
