29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On November 5, 2013 employees working around a crane with elevated loads were not protected by hard hats, exposing employees to a stuck- by hazard. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On November 5, 2013 employees were not protected from a fall while walking/ working from the roof of a commercial building, exposing employees to a fall hazard of 22' 2" above grade. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
