29 CFR 1910.178(l)(1)(ii): 29 CFR 1910.178(l)(1)(ii):  The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a power industrial truck:    a) The Ziegler Tire and Supply Company dba Transport Tire, Toledo, Ohio:  On or about August 5, 2013, employees operated power industrial vehicles to lift and/or maneuver tires and pallets of truck rims throughout the facility.  The employer did not ensure that employees had successfully completed an operator training program containing elements in 29 CFR 1910.178(l) before employees were permitted to operate a sit-down counterbalance lift truck.
