OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an inhalation hazards from hydrogen sulfide (H2S) gas. On or about November 5, 2013, the employer did not provide hydrogen sulfide (H2S) training to the employee(s) working on Sidewinder Rig #17, when working in and around hydrogen sulfide gas  (H2S).

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  On or about November 5, 2013, at rig #17, located at FM 1344, in Wilson County, TX,  there was no eyewash station that provide 15 minute of flushing on the lower level near the hopper. Employee(s) were mixing a chemical called caustic soda. Employee(s) exposed to burns to the eyes.
