29 CFR 1926.20(b)(2): Frequent and regular inspections of the job site, materials, and equipment to be conducted by a competent person designated by the employer was not provided by the employer who was responsible to initiate and maintain an accident prevention program:    a- 5400 Long Beach Boulevard, Holgate NJ: A competent person did not conduct frequent and regular inspections of the jobsite.  Employees on this site were exposed to hazards including, but not limited to fall hazards.  Conditions noted on or about 11/06/13.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:    a- 5400 Long Beach Boulevard, Holgate NJ: Employees, not supplied with fall protection and less than six feet from unprotected edges, were exposed to fall hazards of approximately 16 feet to the ground surface below when applying bondo and resin to the structure's exterior porch/deck.  Condition observed on or about 11/06/13.
