29 CFR 1926.403(i)(2)(i): Except as required or permitted elsewhere in this subpart, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means: a. On or about November 06, 2013 - at the above addressed jobsite, employees were walking/working around a light switch that was not provided with a cover or face plate to protect against accidental contact, thereby exposing the employees to electrical hazards.

29 CFR 1926.405(b)(1): Conductors entering boxes, cabinets, or fittings. Conductors entering boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. Unused openings in cabinets, boxes, and fittings shall also be effectively closed. a. On or about November 06, 2013 - at the above addressed jobsite, the knockouts on the 1900 box located on the side of the temporary panel were missing, thereby exposing the employees to electrical hazards.

29 CFR 1926.405(d): Switchboards and panelboards. Switchboards that have any exposed live parts shall be located in permanently dry locations and accessible only to qualified persons. Panelboards shall be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons. Exposed blades of knife switches shall be dead when open. a. On or about November 06, 2013 - at the above addressed jobsite, one electrical panel was not protected with a cover, thereby exposing employees to arc flash/blast hazards.
