29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:   a) On or about 11/6/13, at the apartment 304, unit D area of the site, Amherst, NY. A pipe threader, that was in use and powered through a flexible cord, did not have ground fault circuit interrupters for protection.   No Abatement Certification Required

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables shall not be used:   a) On or about 11/6/13, at apartment 304, unit D area of the site, Amherst, NY. The electrical cord of a pipe threader was worn/frayed in the area adjacent to the plug. The worn area was covered with a plastic sleeve.    No Abatement Certification Required
