29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    a) Tri State Industrial Floors, Inc. on or about November 08, 2013, at the address of 3654 Libby Rd. Stony Ridge, OH: The employer failed to ensure an employee working from an aerial lift was adequately protected from falls.  The employee working from the lift was not clipped off to the basket while the platform was elevated and/or in motion. The employee was exposed to an approximate 12 ft fall to the ground below.
