29 CFR 1926.251(e)(8): Removal from service: Synthetic web slings shall be immediately removed from service if any of the following conditions (i) thru (v) are present:   (i) Acid or caustic burns:  (ii) Melting or charring of any part of the sling surface:   (iii) Snags, punctures, tears, or cuts:   (iv) Broken or worn stitches:  (v) Distortion of fittings:      a)   A synthetic (polyester) sling used to lift steel beams had a one inch long tear in the yellow surface fabric where it contacts the corners of the steel beams. Extra padding was not being used between the sling and the beams as advised in the sling manufacturer website.         Location:  605 W 129 St, New York, N.Y.      On or about 11/7/2013      NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.753(d)(2)(i): Materials being hoisted shall be rigged to prevent unintentional displacement:       a)  When crane lifting a steel beam the beam tilted vertical and slid through the sling, breaking the sling; the beam was heavy at one end due to extra flanges and was rigged with a synthetic sling as a single leg/ single choke. The employer did not ensure that the beam will remain reasonably level during the lift by doing a test lift, or using a double leg sling, or equivalent safety measures.  Employees were exposed to falling beam hazard.       Location:  605 W 129 St, New York, N.Y.
