29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:   On or about November 06, 2013, and at times prior thereto, employees were working on a rig site, Rig # 2. The railing between the suction pit #1 and shale pit #2 was missing the intermediate rail on both sides of the crossover. The employees were exposed to potential falls of more than 4 feet to the ground below.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:   On or about November 06, 2013, and at times prior thereto, employees were working on a rig site, Rig #2. There were two units on the site which were not grounded properly: 1. The bottom doghouse had a rod buried only six inches in the ground and 2. The toolpusher trailer was not grounded at all. Employees were exposed to potential electric shock and/or burns.

29 CFR 1910.305(j)(1)(i): Fixtures, lampholders, lamps, rosettes, and receptacles located within 8 feet of the floor had live parts exposed to employee contact:  On or about November 06, 2013, and at times prior thereto, employees were working on a rig site, Rig #2. The Parts House light had a missing cover and exposed live wires. This condition exposed employees to potential electric hazards.

29 CFR 1910.22(b)(1): 29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard:  On or about November 06, 2013 and at times, prior thereto, employees were working at a rig site, Rig # 2. The floor going from the rig floor to the doghouse was bent up, rising up approximately 2.5 inches at some points. This created a trip hazard for employees.

In the bottom doghouse, the heater cord was run through a hole in the metal wall. This condition exposed employees to potential electric hazards.

29 CFR 1910.305(g)(1)(iv)(C): Flexible cords and cables used were run through doorways, windows, or similar openings:  On or about November 06, 2013, and at times prior thereto, employees were working on a rig site, Rig #2. At the site, there were two occasions where electric cords were run through metal doors. One was running from the chemical pump to the main fuse box/transformer. The other was at the bottom doghouse where the cord to the interior light was run to the outside through the metal door. The employees were exposed to potential electric hazards.
