29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: a) Job site, 551 Sandrae Drive, Mount Lebanon, PA: On or about November 12, 2013, during sewer line repair/replacement activities, the employer did not ensure that the employees were trained/instructed in trench and exacavation safety. Employees were moving dirt & rock on/near the street next to the job site with no safety vests/high visibility clothing and at least one employee worked in a trench where the spoil pile was on it's edge.
