On or about November 5,2013, the employee working from a JLG scissor lift exposed himself to a 15 feet fall hazard because the chain was not hooked. 29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by a guardrail system, safety net system, personal fall arrest or any other type fall protection:    a. On or about November 5,2013, roof area, three employees installing flashing on a .5:12 pitch roof were not protected from falling 19 feet 5 inches by the use of a guardrail system, safety net system, personal fall arrest system or any other type fall protection method.    Abatement Certification and Documentation are not requried
