29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:  Jobsite, 849 Victory Highway, West Greenwich, RI: An employee was struck in the head by the drive shaft after disconnecting the drive shaft in preparation for movement whereby the employee was not protected from head injury by a protective helmet.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    Establishment: On, or about, 11/04/2013 the employer did not establish a program consisting of energy control procedures, employee training and periodic inspections for towing operations whereby the driveshaft of the towed vehicle was not blocked or otherwise made secure from falling after being disconnected.
