On or about November 12, 2013 and at times prior thereto, at a job site located on Highway 175 approximately .3 miles East of Lake June Road, Dallas, Texas, employees working on an aerial lift did not stand firmly on the floor of the basket and were exposed to a fall of more than 12 feet to the ground below.
