Protection by placing and keeping such materials or equipment at least 2 feet from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary was not provided.    a) Jobsite: First floor, south end - On or about 10/29/13, JR McGee exposed his employees to falling object hazards by not keeping excavated material with loose rock at least 2 feet away from the edge of the excavation.  Employees were installing cast iron sewer pipes.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c).    a) Jobsite: First floor, south end - On or about 10/29/13, a protective system was not used to protect employees from cave-in hazards while installing a cast iron sewer pipe inside the excavation measuring approximately 5 feet 10 inches in depth, 10 feet 8 inches in length, and  3 feet 1 inch in width.

29 CFR 1926.1053(b)(4): Ladders were not used for the purpose for which they were designed.  a) Jobsite: First floor, south end - On or about 10/29/13, employees installing cast iron sewer pipes in an excavation were exposed to hazards associated with excavation operations while working with a ladder that was not used for the purpose for which it was designed.  The Husky 8 foot fiberglass supported (A-Frame) step ladder used in the excavation to access the upper working level was not used as designed by the manufacturer exposing employees to fall hazards.  The ladder was used in a folded position, not opened according to the manufacturer's specifications.
