Panel L3 operates the 110 volt equipment in the facility. b: The branch circuits in breaker panel H3 were not labeled. Panel H3 operates the 220 volt equipment in the facility. c: The 110 volt GE Powermark Plus breaker panel in the maintenance office was not labeled. The panel powers the equipment in the maintenance office and shop.

29 CFR 1910.1200(f)(6): Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with either the information specified under paragraphs (f)(1)(i) through (v) of this section for labels on shipped containers or with the product identifier and pictures, symbols or combination thereof which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical. On or about November 13, 2013, employees used naphtha stored in a clear unmarked bottle, exposing employees to a fire and chemical hazard.
