29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:  On or about July 23, 2012, each branch circuit in the breaker panel was not labeled to indicate its purpose, nor located and arranged so the purpose was evident:    a:  The branch circuits in breaker panel L3 were not labeled.  Panel L3 operates the 110 volt equipment in the facility.     b:  The branch circuits in breaker panel H3 were not labeled.  Panel H3 operates the 220 volt equipment in the facility.     c:  The 110 volt GE Powermark Plus breaker panel in the maintenance office was not labeled.  The panel powers the equipment in the maintenance office and shop.

Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface:  On or about November 13, 2013, a snap switch faceplate on the vehicle lift in the maintenance shop was missing a switch, exposing employees to an electrical hazard.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  On or about November 13, 2013, flexible cords in the facility and maintenance office were used as permanent wiring, exposing employees to an electric shock hazard.           a:  An extension cord was used as permanent wiring to power a three-way splitter which, in turn, powered a toaster oven, microwave, mini refrigerator, and lamp.    b:  An extension cord was cut and used as permanent wiring to power a receptacle box in the maintenance shop to power a battery charging station.

29 CFR 1910.1200(f)(6): Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with either the information specified under paragraphs (f)(1)(i) through (v) of this section for labels on shipped containers or with the product identifier and pictures, symbols or combination thereof which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available  to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.    On or about November 13, 2013, employees used naphtha stored in a clear unmarked bottle, exposing employees to a fire and chemical hazard.
