29 CFR 1910.23(c)(1):  Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:    a)  On or about November 5, 2013 and times prior, employees were exposed to injuries due to falls from a platform to the lower surface when accessing equipment including but not limited to the work platforms of the deethanizer reflux accumulator VS-8117 and ethane/propane product accumulator VS-8118.

29 CFR 1910.119(f)(1): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and did not address at least the following elements (1910.119(i)-(iv):      a) On or about November 5, 2013 and times prior the employer did not ensure that operating procedures for the covered process were consistent with process safety information (PSI).       2) Current operating procedure IGN-OPR-005 dated 10/01/2012 on page 19 references tag number S-8111, equipment list as fuel gas scrubber located on P&ID 5N1-1-634sht1. The location should reference P&ID 5N1-1-634sht2. 29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel:    a)  On or about November 5, 2013 and times prior, effective safe work practices had not been developed and implemented for maintenance operations to address the re-installation of plugs into line after the equipment was placed into service.  A plug was missing on valve V-56 in line of the debutanizer reflux accumulator. 29 CFR 1910.119(j)(2): The employer did not establish written procedures to maintain the on-going integrity of process equipment:    a)  On or about November 5, 2013 and times prior, the employer had not developed and implemented effective procedures and/or effective inspection processes to identify equipment such as piping, valves and other process equipment that has been coated and/or the protective coating has failed. Such equipment was located in and around the debutanizer reflux accumulator VS-8141.  The process equipment, valves and sections of piping showed surface corrosion.
