29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  Location: 46 Keever Pass, Lebanon, Oh  a) On or about 12 November, 2013, Employees were exposed to a fall of over 25 feet, while working a roof with a pitch of greater than 5 in 12, with no form of fall protection in place. Employees were transporting and securing sheets of 4 foot by 8 foot wooden panels to the roof framing.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers.   Location: 46 Keever Pass, Lebanon, OH  a) On or about 12 November, 2013, employer exposed employees to a uncovered opening measuring 57 inches by 4.5 inches that exposed a 16 in deep hole.  Employees stepped over the hole to gain access to the stairs that lead to the roof of residence under construction.
