The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.   A.On November 15, 2013 at Pierce Concrete, Bethany Missouri Facility- The employer failed to establish a lockout tagout program consisting of energy control procedures. During the inspection an employee was observed restocking the sand, gravel hopper and working on the backside of the hopper where the conveyor belts moved the sand gravel mix to the trucks. Employees were exposed to hazardous conditions during maintenance and cleaning procedures with the mixing and moving equipment used during the concrete mixing and loading operations.
