29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.     465 Hartford Pike, Dayville, CT 06241on or about 11/12/2013; Employees engaged in roofing activities utilizing a pump jack scaffold were not using any type of fall protection. The employees were exposed to 26 foot fall.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:     465 Hartford Pike, Dayville, CT 06241on or about 11/12/2013; Employees engaged in roofing activities utilizing an aluminum extension ladder as a platform. The ladder was utilized to bridge the pump jack scaffold platform and the building roof.
