29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   a) 25 Welden Road, Lake Hopatcong, NJ.   Employee was observed working out of an aerial lift basket without the use of his harness/lanyard or body belt, thereby exposing his employee to being thrown from the basket of the aerial lift to the ground below.                                           Violation observed on or about 11/15/2013.
