29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  (a) Roof of residence, 1736 Claredon Way, Eagle Idaho: on November 11, 2013 and at times prior thereto, employee had safety rope attached to the side positioning ring of the safety harness instead of the D-ring.  Note: Abatement certification IS NOT required for this item.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill.  (a) Roof, 1736 Claredon Way, Eagle Idaho: on November 11, 2013 and at times prior thereto, the employee was unfamiliar with fall protection equipment and inappropriately used the side positioning ring of the safety harness to attach to the safety rope.  Note: Abatement certification IS required for this item.
