Location: 369 36st Street Brooklyn, NY  On or about 11/14/13  a) Two employees were installing siding from a 28 foot high pump jack scaffold and they were not protected from falling by guardrails or personal fall arrest system.

Location: 369 36st Street Brooklyn, NY  On or about 11/14/13  a) Employees were not trained  to recognize hazards associated with the type of scaffold being used. The employees were not provided with proper fall protection system while exposed to a potential fall of 20 feet to the ground below.
