29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: a) A written Hazard Communication Program that shall include training had not been implemented and maintained by the employer. The employer failed to implement a program for employees who work with chemicals such as but not limited to polyurethane pellets with the potential exposure to methylene diphenyl diisocyanate (MDI). Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: a) The employer failed to provide the Appendix D located in the OSHA Respiratory Protection Standard (1910.134) to employees who volunatarily wore filtering facepieces such as but not limited to Sperian N95 dust masks. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
