29 CFR 1910.119(h)(3)(i): The contract employer did not assure that each contract employee is trained in the work practices necessary to safely perform his/her job. On or about November 18, 2013, and at times prior thereto, the employer did not ensure an employee operating the a Terex Telelect Hi-Ranger aerial lift were trained on the use of fall protection while working inside of the bucket of the lift, exposing the employee to a fall hazard.

29 CFR 1926.453 (b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). On or about November 18, 2013, the employer did not ensure employees working inside of a Terex Telelect Hi-Ranger aerial lift were in use of fall protection, exposing the employees to a fall hazard.

On or about November 18, 2013, and at times prior thereto, the employer did not ensure employees followed lockout/tagout procedures when they were connecting to existing electrical work and providing temporary power in the facility, exposing their employees to shock and electrocution hazards.
