29 CFR 1926.403(h)(2)(i): Live parts of electrical equipment operating at 50 volts or morewere not guarded against accidnetal contact by cabinets or other forms of enclosures:    315 N. 12th Street, Philadelphia, PA - The employees were utilizing and working around an energized (120/208 volts) circuit breaker panel which did not have a dead front installed on it, thus exposing the employees to an electrocution /shock hazard. This violation was observed on or about November 14, 2013.      Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
