29 CFR 1910.145(f)(3): Tags were not used as a means to prevent accidental injury or illness to employees who were exposed to hazardous or potentially hazardous conditions, equipment or operations which were out of the ordinary, unexpected or not readily apparent, until such time as the identified hazard is eliminated or the hazardous operation is completed (Maritime Reference 29 CFR 1915.91). a) On barge DBL-119 moored at the facility, cargo tank number 1: On October 22, 2013 danger tags were not used to prevent accidental injury after filling cargo tank number 1, cargo piping and vent lines with nitrogen, exposing employees to an asphyxiation hazard. b) On barge DBL-119 moored at the facility, cargo tank number 2: On October 22, 2013 danger tags were not used to prevent accidental injury after filling cargo tank number 2, cargo piping and vent lines with nitrogen, exposing employees to an asphyxiation hazard. c) On barge DBL-119 moored at the facility, cargo tank number 3: On October 22, 2013 danger tags were not used to prevent accidental injury after filling cargo tank number 3, cargo piping and vent lines with nitrogen, exposing employees to an asphyxiation hazard.
