On or about 11/20/2013 employees at the Deforest Street Roofing Project were exposed to approximately 17' falls.  Employees were engaged in roofing activities without fall protection.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  On or about 11/20/2013 employees at the 55 Deforest Street roofing project were using a 50' extension cord that had no strain releif.  The cord was energized by a 20amp GFCI  circuit and powering a Ryobi circular saw.
