29 CFR 1910.67(c)(2)(v): The safety harness lanyard was not attached to the boom or basket when working from an aerial lift. On or about November 20th 2013, the employer failed to ensure that employee(s) were provided with and required to use adequate fall protection. One employee was working approximatley 50 feet high in an aerial lift without having his body harness attached to the basket.
