29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    29 CFR 1910.147(d)(2): Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage.    As of and prior to November 20, 2013, the employer did not require the use of lockout/tagout (hazardous energy control) procedures when performing setup duties. In addition, machine guarding was not adequate for the level of protection necessary when employees must reach into equipment danger zones during job setup, including but not limited to tool changes, for the following pieces of equipment:    a. the Fanuc VT-17 CNC machine,  b. the Okuma Crown L1060 CNC machine,  c. the Fadal VMC 6030 vertical machining center, and   d. the Fadal VMC 4020 FX vertical machining center.

29 CFR 1910.212(a)(1): Machine guarding. Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.     a. On or about November 20, 2013, the FANUC VT-17 CNC machine did not have adequate machine guarding. The machine door interlock was damaged:  in that a contact was missing.       b. On or about November 20, 2013, the FADAL VMC 6030 vertical milling machine did not have adequate machine guarding. The machine door interlock was damaged and removed from the mounting location. The interlock device was broken in that the magnetic component was pulled out or apart from the device.      c. On or about November 20, 2013, the OKUMA CROWN L1060 machine did not have adequate machine guarding. The machine door interlock was damaged:  the cover was missing and interlock electronic components were ajar.

29 CFR 1910.303(b)(2): Examination, installation and use of electrical equipment. Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.    (a) On or about November 20, 2013, electrical equipment was installed and used to supply power for equipment in the "sink area" of the break room and was not protected by ground fault circuit protection equipment.  This electrical power supply circuit includes a combination switch-receptacle box that is located approximately two feet from the sink and provides current for appliances, including a coffee maker.
