29 CFR 1926.451(c)(2):   Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  a) West side of building - On or about November 5, 2013 employees were conducting stucco work and were using a tubular welded frame scaffold to work on the wall and eaves that was not supported by base plates or other firm foundation.

29 CFR 1926.451(g)(1):   Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) East side of building - On or about November 5, 2013, employees were working on multiple sections of tubular welded frame scaffolding that were not fully planked, creating fall hazards from no less than 10.5 feet above grade due to the complete lack of installed guard rails or other fall protection equipment.
